I arrived here November 10 2018. I had and accident and had a total hip replacement. I was brought in on a stretcher from Riverview hospital, and couldn't even get on a bedpan by myself because I was so sick and weak! Within a week, physical therapy had me walking! The nursing staff is great, the Cnas are awesome, the food is good, and they make you feel like you are family! Dr Debora Reiland is my doctor, and I have known her for more than 20 years, and she is wonderful! From the cleaning staff all the way to the administrator of the facility, no body will ever tell you it's not my job! We all work together! I will go home at the end of February or first of March, I am much stronger than I was before I broke my leg and hip! I'm walking, climbing stairs, and getting my body strong for my upcoming liver transplant. I did the work, but the home and staff gave me the tools to get stronger! The place is clean, and the residents are like family. I will miss everyone when I go home, but you can bet I will visit weekly and volunteer after my transplant. I am sorry some have had a bad experience, but I live here, and the residents are treated with love and care. My parents are gone to heaven, but I have a wife and a son that is 8yrs old, and a daughter that is 6yrs old. I would have no problem with them being here for rehab or long term skilled nursing!

Absolutely horrible... Not enough words in the world to describe how horrid this place is. From the ridiculous attitudes and stories that don't add up to the poor people the just leave laying or sitting anywhere with full diapers and all. The state needs to step in and fix this place.

The room my mother is in is terrible. Looks like a jail cell and smells like crap and death. The staff is not friendly at all and never called me when my mother was rushed to the ER. I found out 2 days later.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
