I would give zero stars if possible. My dad's mental status started to seriously decline while here. He came for rehab for 20 days. PA and staff kept saying that's just how he will probably be (in regards to his mental status). Well I know my dad and even though he is elderly he is sharp. He had a Foley and the urine was a strange color. They said no big deal, one of the labs was a little elevated, no problem. Well at our urgency when my dad's eyes started rolling back in his head and he was difficult to wake we demanded he be taken to the ER. HE WAS SEPTIC. If he wouldn't have had family there every day for several hours a day they would have let him lay there and DIE. This place is terrible if you love someone do not send them here. If you don't like someone don't send them here. No one deserves unskilled, uncaring, incompetent staff at a place where you come to get better.

The care seems to be ok,we are experiencing clothes coming up missing, new cloths hanging in her closet, never worn, just gone, and of course no one knows anything. Buy some light weight chain and a small lock and lock up the closet and drawers, give your loved one the key put it on a string and have them wear it around their neck, then they will control who goes in and out of their clothing closet and drawers.

This is the worst nursing home for someone who had a stroke. My mother is inpatient their and they missed a lot of opportunities to get her better. First they told me she was ok after I insist that they have her checked by a nurse because I realize she wasn't mobile, weak, active, slurred speech, confused, forgetful and lost of appetite. I finally ask for them to check her for a UTI and after 6 days they finally agreed to have her checked. I was told that they had lab work done and it was negative. After not getting anywhere with the Nurse Manager, nurse, and CNA. I took my mother to ED to finally get a urinalysis that she in fact had a UTI. They are very unprofessional, liar, and uncaring. Don't take your love ones there unless you can stay with them 24 hours a day.

Do Not Send Your Love One Here! My mom has dementia and had hip surgery and was there to help her walk again. When we brought her home we realized they didn't change her (diapers) like they should and she was SEVERELY raw on her bottom. She had bruises all over her where they (I assume) tried to pick her up or change her. So if your love one isn't able to communicate like they should or has any dementia DO NOT SEND them here. They do not know how to handle them or won't. Believe me Mom is worse now than before she went there.

Night shifts nurses rude and unprofessional. One came out front door to smoke as we're unloading pt. Could hear her complaining about receiving said pt then left us outside in the cold while our dispatcher had to call them to let us in

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
