I can't say enough of how wonderful this facility is! My mom came to this facility after being in a deplorable facility in California. Perry County staff has treated my mom like family. The care has always been up and beyond par, you never walked in and smelled anyone who was soiled, or offensive. In fact it always smelled and looked like they just finished cleaning. The staff would always greet mother with a warm smile and hello and were alway kind and careful. I was always notified immediately when mom had any issues, and the solution or treatment being provided to remedy. The food was exceptional and taylored to patients wants and likes. To the point of remaking a meal to their likings that day. I highly recommend Perry County Nurse & Rehab Center to anyone in need of a skilled nursing facility!

Compare The Number of Problems
Types of Problems at Nursing Homes
Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.
