A beautiful, clean facility. The room is quite comfortable. It may have been a rare mistake, but still one which should not have happened. It was only 3 days after major surgery, I was assisted to the bathroom then left while the attendant answered another patient's call. I won't go into detail but simply state that after a long wait I began to panic, cry, yell for help hysterically. The Emergency cord was flicked on and off 3 times. My legs and feet had gone numb, no feeling at all. I could not help myself, if I tried to stand alone I would have fallen. When help finally arrive and I was returned to a bedside chair I noted 35 minutes had passed. -- l immediately made arrangements to leave the facility;. I could not stay where I no longer felt comfortable, safe, or secure.

My former Mother-In-Law is being cared for at the location of Glendale, Arizona and I wish I could give more stars! The entire establishment is completely over the top and the rooms are so comfy, cozy! The staff are friendly and so very helpful. I'm very impressed and can't say enough. My own Mother was in a "so-called" 5 star rehabilitation back home but couldn't even close to Advanced Healthcare.

Had a loved one that we placed in this facility... We toured the building prior to coming and yes it is very nice and new, however we were very concerned with the lack of attention from staff. My loved ones medications were not on time, cold food etc.

My wife has been here twice in the last couple of years and had always been very impressed both with the rooms and the staff.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
