I recommend this facility to any person in need of care. The staff is kind and courteous towards everyone. Anyone is willing to lend a helping hand and the facility is very clean. I don't have any complaints towards this building or the staff.

Bella Vita is an excellent place for patient's to stay and for people to work. My wife has been there for 11 years now and you can truly see how each staff member not only cares for each other but especially for the patients.

My great grandmother lived here for 2 years on the long term care unit. Her moving to a home was her choice and she absolutely loved it here! As time went on her diagnosis made her less active and unable to speak but that didn't stop the great and loving care she received from every employee. Staff was always kind and if there was a problem they fixed it immediately. Even when she was passing her nurse Ashley was with my entire family and comforted my great grandmother as if she were her own grandmother. Till this day she often checks in on our family to see how we all are doing. Bella vita was just like family to us and welcomed us even when we were a little difficult ;) Thank you Bella vita staff for educating us as well as taking care of her as if she was one of your own family members. She appreciated all that you did for her!

My grandma stayed at this facility for awhile, all the staff members made her feel welcome during a very difficult time. I would recommend this place to any one who needs skilled nursing or even long term care! Also, the activity department is so wonderful!! Thank you so much Bella Vita staff!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
