    I want to visit my grandma more often but this place just reeks of urine. I get sick to my stomach after ten minutes or less. I feel like it shows signs of uncleanliness. I hope my grandmother is safe, it's not my decision to have her here.

    My grandma is here and we have to talk through the smell of urine and feces that suggests that there is something else bad going on. I would not call this a rehabilitation center. It has nothing to help rehabilitate my grandmother and she is not encouraged to do better!!!! I wish I could give this place 0 stars!

    The smell of this place is unacceptable! I want to visit more but I feel like I'm breathing in urine and feces and vomit all at once. You need to get your place clean. When they showed us the place we saw new rooms that were nice and ended up in a room that's not been renovated since the 80's. Gross! Unacceptable. I'd give zero stars if I could.

    Looks can be deceiving and definitely can use some curb appeal. I spoke with Our mothers truely kind caseworker with Thunderbird Boswell(Kris). She spoke so highly of Horizons Acute rehabilitation team. The staff is incredible. Horizons Acute rehabilitation staff started a comprehensive plan for our beloved mother almost immediately before mom arrived. All I can say, is they have given us hope For more tomorrows as a family with mom.
