Oh no! My dad has only been there for a few hours and has already fallen out of bed. They called to tell me he was calling out for me but when I asked if I could talk to him they told me it was impossible for the phone to reach him and when I told them he had a cell phone in his belongings they said they would look for it and call me back. They still haven't called and I am three and a half hours away!

Awful! Terrible place! My grandmother was here and they would not send in a female CNA for cares then the guy who was supposed to help her told her to wipe herself! Whoever you are you should not be CNA! AND THEN during meals they would not assist her with eating. She can't even hold a fork! How was she supposed to cut up her food herself. I'm ready to call state on these people.

If you are in need of intense rehabilitation due to an accident or other medical reasons this is the place you need to be. They have an excellent therapy gym and the only thing I wish they did have is an aquatic therapy center, though they do offer all other aspects of therapy services. I have been here a number of times over the last 3 years here and I have always been able to return to my home fairly quickly due to the excellent therapy services provided to me.

I have been there to recover from 3 different injuries/surgeries. I've always had a pleasant experience and have become good friends with some of the staff. I would recommend Haven to anyone needing a rehabilitation facility.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
