Furthermore, my dad eventually went into their facility for hospice care (a few years ago). It was dingy and dark, the room he was placed in was a disgrace (with even big, unpatched holes in the drywall). I'm glad that he was not in the proper state of mind to notice it himself! Knowing what the family knows after this experience, we should have hired a nurse and kept him in his own home instead.

My dad was at this facility for about three weeks. On the day he was discharged, I called and left a voice mail for the social worker asking her to call me back because I had a number of questions about his at-home care. I called a number of times and she never called me back. Apparently they never sent the orders to the place that was supposed to go out and do his at-home nursing care, which was verified by the at-home nursing care facility. Needless to say, my dad is not in a good place right now. These people don't follow-through with what they're supposed to do. Really sad!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
