Dear marketing people at the Forum: when you give other people the talking points you want them to use on reviews, complete with keywords to tout things like amazing food, Friendly staff, well maintained grounds, you really should mix it up a bit so it's not so obvious.

The food at The Forum is nothing but exceptional. The food is prepared by staff who really care about the way the food tastes. The food is delicious and you will want to come back for seconds. The kitchen staff is always very friendly and takes the time to deliver quality food with a smile on their faces. Food is delicious. Highly recommended.

The kitchen and dining staff go above and beyond here! The food is outstanding and the grounds are clean and well-maintained. I've only had wonderful experiences with all of the staff. They treat the residents and their families like their own.

The forum at Tucson is a beautiful community. The grounds are well kept and the staff is amazing. The maintenance staff went above and beyond to be friendly and courteous when there was no personal care involved. I highly recommended this community.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
