Brookdale has taken care of my loving mother who has just recently passed. The staff at Memory Care was excellent they never missed a beat with my mother. I have nothing but high praises for ALL the staff in Memory Care!! They always had something going on AND Victor the piano player & singer & was my mothers favorite she loved to hear him play & sing ? I never was disappointed? And most importantly, I NEVER heard any of the staff say one one bad word about the residents?Kind, extremely patient, & calmly they care for each resident...I wouldn't have my mother any where else?Thank you to all the staff at Memory Care ? We will miss our mother ?

During my 97 year old dad's stay at Brookdale Carlsbad, he experienced the wonderful, loving staff's attentiveness and responsiveness to his every need! We were so grateful for their caring ways showing outstanding service with respect and compassion. There were always staff members available to him. He had just come out of the hospital and we feel he couldn't have gone to a better place to meet all his complex needs! We give you 10 stars! Great job! Thank you!

My mom has lived at Brookdale Carlsbad for almost a year now and the care she receives and the staff are wonderful. Today my family had Easter brunch with her and the food and service were excellent. Even my sister-in-law, who by her own admission is a very picky eater, finished her plate. It was obvious that Marco, the chef at Brookdale, had put a lot of thought and effort into the menu. Everyone at Brookdale is so thoughtful and caring and I feel very comfortable having my mom live there.

My wife needed some recovery time from back surgery and a friend recommended Brookdale. We are so lucky she new about this place. It is unbelievable, the whole staff puts the patient first with loving care. Don't ask me to name one individual because then I would have to name every employee. I have eating meals with my wife and spending the time really helps me get thru this. Thank you Brookdale, we love you

My father in law has been at Brookdale Carlsbad since September of 2015. He has received excellent care the entire time. The staff is sensitive to our family and to his needs. Thank you to everyone for helping us during this difficult time.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
