Can't say enough great things about this facility. Working in the customer service industry myself, I greatly appreciated how professional and friendly the staff was. Such amazing people! Plus the facility is odor free! I highly recommend this facility to anyone.

When I toured the building I was happy to see that it was clean & bright. Most importantly, there was no odor in the halls or rooms! The gym for therapy was large & the staff looked happy. Would definitely recommend this place to anyone needing a skilled nursing facility for their parents.

this place is a dump. It used to be well-kept under the prior owner and director and activities personnel. I recently had the opportunity to go inside and was appalled. It was cluttered and dirty. The owner seems to be in it for the money, not because he cares. The good people left because either they weren't getting paid or because of the way they were treated. The current hairdresser is a butcher and I don't know if they have a manicurist anymore. My sister is there and I've never seen her look the way she did when I saw her. Her hair hasn't been done in I don't know how long and her fingernails are filthy, possibly fungus growing. Not sure. Unfortunately, I do not live in Sacramento so that I could be an advocate for her. This place is absolutely pathetic. And should this be read by the owner and anything happens to my sister, I will know it's because of this, so watch yourself, Mister.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
