I searched reviews for the best place. So dissappointed this place hid negative reviews by changing their zip code and location originally it was "Laguna woods" there in google searches it has many negative reviews. The new zip code has almost all good reviews. I don't know who posted them but they r not correct. It is not modern, it is dated. Most importantly it is extremely dirty. If it was a one star hotel it would fail average cleaning standards. Shower stalls are filthy, fixtures don't look like anyone has cleaned them for months. The white in the shower stall is deep grey. The toliet / shower seats underneath for the elderly were caked with old urine. Curtains in the room were soiled and dingy. Metal hand rails are pulled away from the walls. Exposing inside walls. Bathroom doors were soiled with yellow blotches and dirt. They need a serious deep clean of all surfaces. I saw hairs stuck on walls and floors. Wipe at almost surface with a clean wet wipe and it will come up grey. Air conditioners were thick with dust and hair. Not good for many older people who have breathing problems. Sinks are dated rusted metal hands very small and a challenge for even able bodied People to turn. The shower heads also were old and need cleaning or replacing. As well as the shower rings and curtains. I suggest you hire a maid service to deep clean and sanitize the room your loved one is using. Clearly what they think is clean is sorely lacking. I hope to see a change on my next visit but its clear its been that way many many years. Its not new dirt. Doors are covered in yellow blotches and drips which shows they just havent been cleaned evwn on a weekly basis. Next time I'm bringing a black light. They bill at a very high rate there's no reason why they can't have a dedicated cleaner. Google their original reviews for the previous Laguna woods address. Street and number are exactly the same. Also visit. The location. Don't just drive by. The actual entrance/building is hidden behind a much much nicer cleaner apt building. This building doesn't have automatic front doors. Speak up or poor conditions won't change. Search further and other site show 2.5 star average. They need to learn from the reviews not hide them.

The Palm Terrace at 11162 Palm TerraceLane in Riverside is a nice place. Clean and competent. Our patient is mentally very sharp, so she is not as thrilled with it as we are. The reason i didn't give 5 stars is the food. It is pretty awful. The facility and staff are terrific and attentive, and the grounds are nicely kept.I also have to add its quite out-of-the-way location.

Palm Terrace does a great job of working with rehab and long-term patients. I know the administrator, medical director, and nursing director there--they're conscientious, caring, and concerned about making sure their patients get rehabilitated quickly and back to their lives.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
