I was given my medication in the darkness of my room I noticed I missed my 12 a.m. (it was 1 a.m.) medication dosage when I took the pills I noticed the texture and taste so much different, although she left right away (the LVN) and told me mrs. blank here is your medication and left. When I took the medication out of my mouth searched it on my phone by the imprint, it was half of my normal dosage of the same medication is this even legal?? when I called the nurse and asked her why she did that she says that she told me what I was getting I know for a fact she did not tell me. I don't know what else to say. This place lies to you and gives you the wrong medication and the wrong dosage. Be careful people be very careful. Know what's being dispensed and advocate for yourself. God forbid you have an elderly parent or grandparent in here who knows what they might be getting and what you're getting billed for..

There is obviously a section about f care that is picked for the client. My mother ended up in the hospital twice while in their care. Due to negligence. When I said something was wrong with my mom days earlier than her ambulance ride into the ER. First Es Tim she was so sick it took 3 weeks of hospital care to turn the infection around. Second Tim it was yet another week. They fitted my mother with the absolute wrong equipment for her bipap.only using a nasal mask instead of a full facial mask. Took a week and move out to another home to fix that. You should not have to have the physical therapist call me to say. Hey I think your mom is not well. WTF. HERE IS THE OVERALL PROBLEM. Unless you have millions of dollars to light in fire and burn to ensure the patient has a good stay. You are screwed! They released my mom today to another nursing home. What th no medications and no glucose tester. And to boot. They were not ordered at the pharmacy either. Still waiting on it. It's been 4 hours. NOT IMPRESSED

Very rude and unhelpful receptionist. If the rest of the staff is that way then I would never send my grandmother here.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
