What can I say. It's a actual Skilled Nursing Facility. Staff seem nice. Parking is in the back and be careful when you're looking for it because it's kind of hard to see from the main road.

This facility is not clean ,the language barrier is also a problem, the nurses seem nice enough.but this place is filthy....they evening shift is aggressive with patients when no one is around, when they think no one is watching.they let patients walk out the front door when they have a patio to go out on,and the leave patients in the dinning room all alone in the dark!!!! This place sould be shut down.........

Redwood Healthcare Center may not be looking as fancy like other facilities surrounding it, I'd still give the highest compliment to the team that provides the best care to the customers; the residents and their families.

Untrained staff, many with significant language barriers, offer greatly reduced levels of care. Building is conspicuously non-ADA complient. Client restrooms not usable by those in wheelchairs. Wheelchair-bound clients many forced to continuously dishonor themselves due to lack of essential equipment if you are not going to have bathrooms that are wheelchair accessible. Occasionally unclean food means that some residents if they're able must find food elsewhere, due to never knowing when the food is going to be clean. Even if the food was clean, it's probably healthier to eat the paper plates they serve every meal on. Upon my arrival to this place they didn't have any of my very necessary seizure medication or medication for PTSD ordered I didn't have any medication for the first 48 hours that I was here. My seizure medication wasn't filled properly for 6 weeks. At 7 weeks I finally was able to sign some of the admission papers as none of it had been done at all. They are frequently short-staffed constantly calling on call nurses and CNAs who don't quite know the residence and owe me the merest scratches of an outline of what the job entails. Resident call lights can go unanswered for up to two to 3 hours at a time. The state of care can be quickly assessed by looking at the state of the clientele. Even for a nursing home in Oakland, the state of this establishment is extreme. If you have the means and the ability I highly suggest if you have serious needs or have a serious need for rehabilitation physical rehabilitation as I do you will need to find another place to receive your rehab.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
Redwood Healthcare Center LLC has not received any fines, complaints, or inspection problems in the last 3 years.
