    Dirty, the smell is horrid, staff doesn't make sure patients are hydrated or fed if they have problems feeding themselves. They leave urinal bottles too far away from patients causing them to urinate the bed. They handle patients with bone cancer very roughly causing possible fractures. The cna's are rude and don't care about the patients comfort. Horrible and very horrifying place!

    This place needs to be shut down. If you care about your loved ones please do not leave them here. This place almost killed my mother in law. They are negligent and only care about collecting insurance and money. My mother in law was admitted to the hospital from this place half naked and with a blood sugar over 700. She is blind as well as type 2 diabetic they were not checking her blood sugar daily. They claimed they checked it once a week. I couldn't believe my ears when they said that, the hospital informed me that her body was septic and her kidneys were failing due to severe dehydration. She was so close to death her blood pressure was 44/36. Thank God she was strong enough to pull through and she is okay now but this place was so negligent that it almost took her life. Every time I would visit her her clothes reeked of urine and feces she smelled of body oder as if she had not been bathed in weeks . I made the decision to remove her from that hell hole and bring her home were i knew she would receive proper care, but they made the process so hard. They told me they would have her home in a couple of weeks but I ended up battling these people for 5 months trying to bring her home. They kept giving me excuses about her insurance and medical equipment needed as to why it was talking so long. Meanwhile they are neglecting to care for her and her health is declining rapidly because they are refusing to let her come home. Then she ends up in the hospital nearly dead. ? well come to find out the hospital informed me that I could have requested to sign a form that would relinquish her from their care and she could come home despite what they say. The hospital also informed me that plot nursing center now known as providence nursing center because they changed their name recently knew this information and yet they refused to inform me of it. The social worker at the hospital said that they withheld that information intentionally and that it is a secret they don't want people to know. The reason I am sharing my story is because I feel everyone has a right on know that vital information just in case they find themselves in a similar situation. Please people don't leave your family here you will regret it. I have already reported them to the ombudsman and hopefully this place gets shut down before they can hurt anyone else.

    My mom's mom,uncle's grandma and husbands grandma all passes due to negligence.. And the roaches say it all aside the fact the people wash all the clothes together n don't give them back to the patients.One time my aunt wrote my grandma's name on her socks, when I came to visit my grandmother was wearing another patients socks. Horrible place. Don't put your loved ones here if u really love them???????????????????They should get shut down.
