Notice how this place only has 1 picture of the front of the facility. That is because it is a disgusting house of hell. I do not understand how they are allowed to be opened it should have been shut down many years ago. It is the filthiest smelliest and rundown facility. You also almost feel like you stepped into a time machine when you see how the place is so outdated its dangerous. But the filth and stench are the worst!!! Now, if that was not all, my children and I contracted STAPH while visiting my husband there. We last saw him on a Sunday. We returned to visit him 1 1/2 weeks later on Wednesday. My daughter hurried ahead of the rest of us to see her dad. She ran out crying because, her dad was not in his bed and she believed he had passed. What had occurred was that my husband had been sent to the ER the Monday after we saw him due to Staph. This facility never notified me "his wife" nor anyone else of kin. I hope and pray it is shut down soon. God have mercy on the poor patients and residents. NEVER TAKE A LOVED ONE HERE!!!!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C

Staffing Levels Per Resident per Day

Medicare determines the expected staffing time per resident per day depending on level of care the residents of Mission Care Center require. It is important to compare the reported time to expected time for a single facility instead of comparing the amount of time per resident of two facilities. Learn why.
