Ms Luu and Lien love this place. My mother is having cancer . I have been coming to Plum Tree almost everyday and staying half of days till midnight by my mom. I saw a lot of things and want to be fair to Plum Tree Care Center. They provided excellent service and good food, snacks , entertainment to all patients. We were here for three weeks and now we are coming back for 2nd time. Service is still the same. I love the manager, admission employees , social worker staffs, nursing director, front desk staffs, RN, CNA, Ot, PT. The place is clean and provides refreshments all day to all. My mom missed her fur babies and I was able to bring her dogs in so she could seen them and of course with proof of all shots. I have been to other nursing skills facilities and Plum Tree Care Center is the best of all. My mother said this is the only place she like to stay. Highly recommended this place. Thank you to the owner for hiring great employees.

I am Sowmya. I worked in the Plum tree Care center for five months, daily two hours from Monday to Friday. It is an excellent center with clean environment. Staff and Volunteers are very caring, loving, affectionate and helpful in all aspects to the residents and visitors.

This is a very nice community. I would trust them with my own grandparents because of how nice the staff has treated me. I would highly recommend them to anyone!

Well, for the second time in a 1 2/2 year period, mother had a major fall and ended up in ER and ICU and hospital for 2 weeks. When the time came for selecting a place for her rehab period, I chose Plum Tree, again, without any hesitation. It was the right choice for us and my mother ended up staying at this facility for 4 weeks. A lot of the staff remembered my mother from her first stay last year (The retention of most of the staff over a long period of time is by itself a sign of good, caring and well managed organization). From the admission office, to front desk, ,nurses, nurses assistances, the activity team, the kitchen, housekeeping and maintenance staff, everyone treated my mother and I (And all the other residents and guests) with the utmost care and respect in a clean, well managed and well organized environment. I hope that you never have to stay at a rehab center and live a full and healthy life, but, if you or a loved one is in need of a place to rehabilitate, Plum Tree is as good as it gets. Thank you for all your love and care for my mother and I ;-) B. Yeganegi

Plum Tree provided an excellent service to Mr.Valani. He says that the nursing staff as well as the physiotherapist were extremely cooperative. They respond to the patient's needs at the click of a light. After 2 weeks of knee replacement he was able to stand on his own without a supporting device because of the intense work of the very good physiotherapists. I would definitely recommend this rehab to everyone!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
