I did community service here. I played duets on my flute with my private flute teacher. We loved everyone here! The elderly here were such a wonderful audience!!

This place does not even deserve one star. If you love your family member do not bring them here unless, like us, you are desperate. They are lazy, unskilled and just want to drug your family member so they don't have to work hard. They waited until 11pm and patient dumped my mother so I could not reach her doctor. They also lied to a doctor to get the order they desired and told him her medications didn't work. The hospital said the medications did work. These people are the worst example of Elder Abuse you can ever find and watch out for Rebecca, Cassie and Monica who are lazy and willing to lie to you. I intend to have their licences revoked for the manipulation and deceit they engaged in. They should not be allowed near old people just like animal abusers should not be allowed near animals. I cannot stress enough their level of abuse.

Staff, food and surroundings are wonderful. it's the skilled nursing we had issues with. Failure to followup with contracted pharmacy resulted in medication not given as ordered and on another occasion doctors written orders were not followed as they should have been.

Wife needs oxygen 24/7 to live. I visited every day. One day I came in and she was completely out of oxygen. I immediately went to the front desk to tell them, as the alarm over the door was ignored. Of course it was fixed. Of course I was angry ! The point is, no matter how much you pay anywhere, you better have the time to oversee the "care given" and have a family member there as often as possible. DON't trust anyone ! This maybe deleted, but I have been honest.

My 94 yr old mother was a resident from Mar 2016 until Feb 2017, when we moved her closer to family. I give high marks to Brookdale for their love of my Mom, the quality of food, housekeeping, activities (Roger and Hari, you're the BEST!)and their personal service care to my Mother. Their Skilled Nursing Facility is the best in the area, my Mom spent several months there undergoing rehab, they are good. I recommend Brookdale San Juan Capistrano.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
