Unannounced tour of the building during breakfast. I was pleasantly surprised to see the staff & residents happy and smiling. The building presented well with no odors, very clean. All staff housekeeping, maintenance, dietary, licensed & department managers were amazingly kind & caring.

The LVN's there are very rude and have no knowledge of the medical field. Those taking care of my Grandpa have been rude lately, now he is dying there since they dehydrated him then they over medicated him. Also they had left him soaked on his bed for a whole day in which my family had to change him since no one else would. The light to request for help is always since they don't turn only ours off so they don't come and help if there is help. He had almost fallen off the bed so my grandma who speaks Portuguese and little to no English had to scream down the hallway for attention. Now he is being transported back in an ambulance to Doctors in Modesto again (stroke patient). Do not go here unless you would like to die there in the smell of pee and bad staff.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
