My parents were residents, on Mountain View side, for 3 years, and for that time, they were treated with respect to begin with, and ended with the staff being family. The care they received from the nurses, cna's, kitchen staff, social workers, and other employees made them feel comfortable and loved. There are many activities for the residents to be involved in and they are encouraged to do so. It is clean, comfortable, bright, and well lit. The days before my mom passed away, they came in often to talk to her, made her comfortable, and not alone. The day she passed, away the staff was supportive to my family and treated her, and us, with respect. I highly recommend North Shore.

My dad stayed in North Shore for rehabilitation after a fall. The staff was awesome: loving and caring. Everyone was friendly. We'd been in a couple of nursing homes before, and this facility really opened my eyes to what a "good" nursing home should look like. Upbeat. Positive. If you or a family member need care, I would definitely recommend North Shore Health & Rehab.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
