This is a beautiful place to live. The grounds are lovely and well-maintained, the staff members are considerate and helpful, and the residents quickly become friends with each other. Individual apartments are large and comfortable, and residents enjoy decorating them with their own favorite furnishings. There are beautiful guest rooms as well, so that out-of-town visitors can be conveniently housed while they are here. The calendar is filled with activities for everyone, according to each person's interests. This is a great place to continue to grow in spirit and to enjoy Life!

My parents moved into Forwood Manor in August and they have been treated with kindness and respect. Jamie and the rest of the staff have gone out of their way to make their new home, feel like HOME. There are many opportunities to get involved, and the residents are very friendly and welcoming.

This is a beautiful Senior Living facility. I toured it and found it to have a great atmosphere and very happy tenants.

I had the wonderful opportunity to visit several of my friends in Independent Living last week. Upon entering the dining room I spotted my friend Brad with a group of his friends in happy conversation and enjoying lunch. Their server could not have been more accommodating to the group. What great piece of mind knowing that Brad is in such a wonderful caring environment! Thank you!

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
Forwood Manor has not received any fines, complaints, or inspection problems in the last 3 years.

Staffing Levels Per Resident per Day

Medicare determines the expected staffing time per resident per day depending on level of care the residents of Forwood Manor require. It is important to compare the reported time to expected time for a single facility instead of comparing the amount of time per resident of two facilities. Learn why.
