My mother was only at Avante Boca for about a month before here passing. She was treated with the utmost of care. Even though my mother was in poor condition. Avante staff did everything to make her comfortable. Anytime they gave her different medications, changed her rooms, address her wound care or taken to the hospital, I was promptly notified. They were also very honest. Several nursing homes denied my mother's admittance stating she had no more days left through Medicare and that I would have to pay outer pocket costs. Avante did a check and informed me my mother had over 70 days of coverage remaining. The other nursing homes were out just for the money but not the care. Everytime I visited the facility I was greeted with a professional and caring approach to all my concerns. Two people I would like to send a special thank you Ms. Sardis Mendez, and Ms Rosalyn. They really went above and beyond to address my mother's care. Avante Boca I would definitely recommend for your loved ones care. Thanks Avante Boca.

If I could give this place no stars I would they are NOT responsible at all they beat people and everything. I am 10 and yes I am saying this. I went with my mom, uncle, and aunt. They went to the cafeteria while I went to go see my grandma because I always go and see her first before anything, so I went to go see her there were no nurses. From what my mother had told me she had said that my grandma had told her that she asked for the nurses to help her to use the bathroom, but they didn't do that because she was shaking way to much but that's for her.The nurse had checked her blood pressure while she was having a seizure and just left her like that so this makes me think what really goes on in their. Also keep notice that 10 people have died there in a month.remember I'm only 10 no hate I'm just stating FACTS. If I were you I'm 10 turning 11 Do NOT I repeat NOT put your family or friends in the nursing center. My grand mother was left there like they didn't care they didn't do anything and that's why you shouldn't put anybody in that center

I forgot to add that the director for nursing is nasty to deal with She looks at me like a piece of sht. she only cares about money. Avante should make changes if they want to stay in business.

My Uncle is currently being cared for at Avante and I've been very happy with the staff. For example, they have frequent calls with me (live in Boston) to update me on his status. They're very responsive as well and have taken great care of him.

I am new to the area (moved from CT) I started calling several nursing homes looking for work but nobody had a FT position or they did not talk to me over the phone. They just said look online and apply...RUDE! When I called the HR department at AVANTE about a job. the HR lady (I forgot her name) was so wonderful. She explained what was available and how to apply. She helped me with general information about area This is something that she didn't have to do but she did because she cares. I will be applying for a RN job as soon as I get my FL license. Thank you for all your help

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
