I recently visited a resident to deliver yarn and I found everyone to be very friendly.. The lady's room was clean and she was very happy there. I would recommend this facility to anyone needing rehab care.

My experience with Madalien, your Director of Admissions was a very pleasant one. She was very knowledgeable & able to answer all our families questions. She made us feel very at ease about the care our grandfather would receive at the facility. Unfortunately, our Papa passed before we were able to make our decision. I would recommend Bonifay nursing home to anyone needing care for a loved one.

I have had such a great experience with this facility. The care and compassion every time I have spoke with Madalien has been so great. She really cares. She was really supportive when I met her at the hospital And when I went to see my family member on the weekend she was there in scrubs working. Amazing to see as I work in healthcare myself.

Excellent staff and care! Madalien was super helpful guiding us through the admissions process. If I could give 10 stars I would!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
