Don't send your family here. My Mom was neglected, abused, and died in hospice within 7 months of moving there. They settle the lawsuit with my big sister behind our siblings backs, and sealed the case.

Don't take your family member here. I am in the process of getting my mother out of there now. They are very abusive to her, talk any way to her, food was aweful I have five witnesses on how this can talked to her we were there and a night shift guy told her to have her tv off by 11 or he was yanking it out. Food is aweful

I would not send my animals here. I was a patient for 9 days. Filing with the State, and my atty. Is busy. The foul stench inside says it all.

I do not understand why it is so hard to get ahold if a patient or someone that doesn't disconnect your call or get transferred to a machine. Not happy

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
