Louise Mucci and Miss Prudent were the best. Actually the whole staff was awesome. The place is very clean the nurses are very Attentive to the patients. The therapy is unbelievable I am very happy that I got to have my dad spend a week there and was treated like a king I rate this placeNot five stars at 10 stars.

My experiences have been a nightmare. I would not recommend this place to anyone. Im having a terrible experience and im unable to help my friend because i have no legal rights. Be aware of everything you do. Very little attention is given unless you stay on top of it. I watch my friend go days without being washed. I said something about her smelling and that she needed a bath. I returned three days later and she still had not had a bath. I was not happy. Within two days of her being there they put in a catera so they would not have to change her diapers within a week she suffer suck an 8nfection it nearly killed her. After several week in the hospital on life support she manage to pull threw and was return to this place. I dont think they liked me because i truely cared for my friend and was there every other day. They now have power of attorney over her and now refuse to allow me to see her. So no i dont recommend this place for your loved ones

If you are exhausted of dirty, filthy, and irresponsible staff of other nursing and rehab centers do you loved one some justice and get them in this center ASAP. My father was here a littke over a year ago and the place is kept clean and the patients are kept clean and well fed. After 3 other nursing home experiences from hell Aventura Plaza saved my father's dignity and health. Please do not waste valuable time and get your loved one into a respectable and accountable establishment. Best wishes and much love to all the staff and doctors of this great institution.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
