If you love your family. Look somewhere else!!!! They are disorganized- my grandmother never got her admission or discharge paperwork done until weeks after she left and when we got it, even her name was wrong. They do not follow through with services. They do not return calls of concern. My grandma was warned about this place, but because her church did service there, she went with it - now she wishes she hadn't.

Don't know where to begin, from the moment you walk in, everyone is lost, there is no organization or communication. Mind you there were like 100 employees there, in groups of 4-5, all talking about lunch, I guess they work for lunch, no one concerned of any patient. No one interested of the people standing around asking for help. There were over 6 patients screaming for help. Meds were left to give to my elderly for 5 days and he came home with meds left for 3 days. I guess medication is not important. Went in that he could eat and came home that he can't swallow and don't know how to chew since they just dropped food there and picked up full plate 1 hour later. I spoke with social worker from here after 2 hours of the elderly being there and she complained for me to pick up elderly that the elderly was walking around? I then spoke with the nurse and she had no idea that I was called apparently social worker was not feeling him being there (don't know why). I asked if meds were given, food, etc. and I guess I asked her the million dollar question due to her response which was birds chirping in background. I couldn't understand her complaint. I guess they want all patients to be bed bound? This is a facility for elderly people? Another nurse said that my elderly went home and was discharged and they were sitting at nurses station all day with no food. I had to call two different people to see where my elderly was and what is happening. I was given a heads up by someone that does business there and said that it was very distasteful what he saw there as a place of business. If you love your elderly I would suggest to think twice before leaving here. No communication between anyone! Very unprofessional from ADMISSIONS forward. Everyone with a bad attitude. No one knows anything! Disgraceful ??? think twice before leaving here to rot! Whoever is in charge should step back and look at the whole picture from the outside. Be an UNDERCOVER BOSS because this reflects you! Shame on YOU

My Dad was in this facility for 2 weeks, and thankfully we had a family member there each day to track down help after call light waits were as much as 1 hour and 15 minutes, and to inform nursing staff of his health status when they didn't know what should have been documented in his charts. We were told by the nursing supervisor that her nursing staff knows everything about each of their patients, and we witnessed firsthand that is not true. We made the decision to move my dad to a different facility, and that decision was not received professionally by Hawthorne's administration. I'm grateful he is now in a great facility where we have confidence he is being well taken care of.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
