My mom stayed at Avante for rehab recently after being hospitalized due to a fall at the house. My mom was despondent when arriving. I too am familiar with these types of facilities because this is not the first incident for my mother. Nor the second. Therefore I was cautiously optimistic. However, after the first night my mom had nothing but glowing reports. I too found everyone to be very accommodating and professional. The Avante facility has been in town a few years and that can't be helped but my mom actually had rave reviews about the food and facility. I've never heard her say that in any of the places she's stayed. The place was kept clean and the medical professionals and staff were on top of everything. Someone told me there was new management. Regardless, they are doing a fantastic job!

This place is very nice. It's in a very nice location in Ormond beach. My bf's dad had to stay here for awhile after a heart attack. The doctor sent him here to recuperate and for some therapeutic assistance. There were only positive things that he experienced.

BEWARE Ask anyone at Ormond Hospital this is where all Avantes patients go. They hate Avante and can tell you one thing after another. I have a grievance sitting in there right now. I had to call over and over to get addressed. So no you dont discuss things immediatly. Even when I called corporate no call back. Staff opens up sanitary items and leaves them open until time to use all over the room. Which explains everyone getting sick. Lets open this sanitised catheter and use it in 2 days wonder if the patient will get a infection. Not enough staff and so staff do things out of their job description which is illegal. And very dangerous. You cant have arm tattoos and work at McDonalds without covering them. Half the staff look like theyve done prison time with prison tattoos all over. No standard of professionilism here. Everyone has a different answer to medical issues of your loved one and when the heck are the doctors there we never saw one. So how are you going to know how they are doing with no real answers. Dark dreary rooms would depress anyone. A coat of white paint would help. Maybe theyl put your loved one as far from a window as possible like they did ours. Literally I needed prozac after just visiting imagine living there. No boards on the wall so staff comes in and puts the bad leg down good leg up. Because they dont even have a board or anything. I thought it was funny the cleaning lady had a clip board but not the nurses. They kept saying they were put something on our loved one at night so we showed up at 2 AM of course they were lieng. They dont talk to patiants who are blind or bedridden to tell them what they are doing just do it. Ive been told its the lowest paying place around and if you get fired elsewhere you can get a job there no problem. Wed give our loved one something to hold onto and theyd take it from them soon as we left. I dont know what the owners talking about changes except for they were almost stopped from being able to take Medicaid & Medicare by The State yet they still do even though their recent offenses found from the state would make you question why. I would call the cops if I saw some of these people around my kids yet Im supposed to trust the life of someone to them. Bedsores like crazy their which is neglect period you cant spin that. And almost any lawyer in Florida will jump on just a bedsore case. At night theirs very little staff even more than the small amount during the day and the weekends a free for all as management isnt there. Thats when it gets extra dirty to more so than usual. Bring your own pillows chairs etc. For you and your loved patient as they are hard to come by there. ===Stop by and read the big book in the front the one about all their violations from The State you cant miss it its huge and across from front desk====

This place is absolutely disgusting!! My father was in the hospital for three weeks and was moved here stupidly by uncle. The first day I go to visit him he's not on any machines and is completely out of it. I ask why and they say because he didn't want to be on any machines. The man is completely out of it and they're going to ask him?? When he has a poa! They also try to get a pencil in his hand to sign a do not resitate when my uncle the poa already said no to that. I asked how his vitals were and the nurse feeding him laughed and said "I don't know." She even went as far as to laugh in his face and say "would you want to eat this food, this is disgusting." I could not stop crying the whole visit. It also smelled so terrible in the place and my poor dad was left there. He didn't even last 24 hours in there because of the poor supervisor of this place. I work in the legal system and am currently working on suing this place. It should be shut down immediately!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
