The worst place to put your loved ones .. Staff is Horrible .they treat old people like animals.. they are dirty , once you go in the facility all you see is these poor people in wheelchairs in the hallways just sitting there .. they DO NOT TREAT THE PATIENT WITH CARE. nurses are not carefull at all. My dad has arthritis and one of the nurses just pulled on my dads legs so hard my dad was screaming in pain.. nurses have an attitud issue .... my mom was crying for her father and one of the nurses laffed in her face and said.. why are you crying we treat them good... my advice.. if you really love your family member DO NOT ..I REPEAT DO NOT PUT THEM HERE..TAKE THEM HOME AND COMFORT THEM AT HOME....THIS IS NOT THE PLACE... THEY FORCE THE PATIENT TO SIT IN A WHEELCHAIR IN A ROOM FULL OF ELDERLYS FOR HOURSSSS AND IN THAT ROOM ALL YOU HEAR ARE PEOPLE IN PAIN . MOANING ..I HATE WOODBRIDGE I SPENT THE WHOLE DAY WITH MY DAD AND WAS NOT PLEASEANT AT ALL...

It's an honor to be apart of woodbridge team ! I enjoy helping families place their love ones rather its short term rehab or long term care we cater to our residents needs !

My mother in law was recently discharged home from Woodbridge Care Center and is doing well, thanks to the excellent rehabilitation and nursing care she received. Before her stay she could not walk or perform any activities of daily living without assistance and she is now living independently with weekly home health visits and a walker. The staff was kind and helpful which helped with her confidence and motivated her to get stronger. Thank you Woodbridge staff for providing excellent care!

My father was recently discharged from the VA hospital to Woodbridge care center and it was the best rehabilitation my father could have received ! The marketer came right away to complete the assessment , she was very polite and informative and my father was admitted the same day into a VA suite and was apart of their VA program . I would highly recommend Woodbridge for rehabilitation and nursing care .

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
