While I was there after neck surgery, I found that the staff was wonderful and most caring. The sure couldn't have changed THAT much. I may have to go to rehab now, and THIS is the only one I would consider for in-house treatment. They have everything you need to rehab correctly and successfully.

My Mother was transferred to this facility from Florida Hospital .Zephyrhills on 3/1/17. Once she was settled the care was not the best. I understand they have a lot of patients to deal with. My family and I where with our mother at least 12 to 13 hours a day. It took a week to see a Dr. We tried several times to get questions answered and where continually told to check with someone else. After getting the run around too many times my sister and I spoke with Christina Cacciatore Director of Nursing and Richard Kulmeyer Administrator.. I was promised a call back from Christina the next day Never received a call back. My mother's right leg was cut by the wheelchair leg rests once by herself the second by a nurse, Christina Cacciatorre RN did not want to treat the wound properly. Mom had to to to the hospital for a surgery related issue, the ER kept her. I notified the rehab center she would be in the hospital overnight and asked if there was anything we needed to do with her personal belongings. I was told again they would call me. The hospital found a lot of infection going on at the surgery site and kept her, when I notified the rehab center they had already packed up her belongings without a phone call to let us know. My mother HATED IT THERE. I do not recommend this place for the care of your loved one. They are very unorganized and give you the runaround.

Our mother was a patient in the facility for 5 years. We were always pleased with the care she received. However when she passed away we received a cold telephone call asking what they should do with the body. None of the nursing or administrative staff contacted us to offer condolences. To add insult to injury they threw away all of her personal effects without letting us know. We are extremely disappointed.

There is this black male nurse that didnt check on a Dialysis resident at night. Resident was found dead the next morning from his dialysis port. You have to monitor things like that. Cnas smoke in the laundry closets, where the clean linen is kept. They do not toilet resedwnts leave them sitting old pee. The ex D.O.N. was covering up a lot of issues.

Zephyrhills health and rehab is a clean and beautiful facility. They have very educated and well trained staff at this facility. Your love one will be well taken care of and receive wonderful service from all departments. They will treat you like your family. They have daily 1 on 1 therapy department, 24 hour nursing care, and daily housekeeping and laundry service. You also can have a select menu from dietary. I hope you will plan your rehab here after any surgery, knee replacement, hip replacement, or any other medical need.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
