Poor rating! I give them 1. I'll keep it short but I could write a book. My dad was here 6 months and it was a bad experience the whole time. The nurses are not compassionate at all!! The whole time my dad was in here they never informed me of what was going on with him in fact when I would ask most of the time they would look at me like I was crazy. They kept my dad over medicated most of the time . At one time I had to demand my dad be taken to the hospital cause he was just sleeping and he was dehydrate!! Unfortunately I didn't in time this time because my dad passed in here July 25th and I know deep in my heart it's from being over medicated!! When he got out of the hospital at the time this was the only place close enough for me. And I tried getting him moved to another facility. What I know about this place I would not recommend this place to anyone!! I went every evening to feed my dad cause I knew I needed to keep my eyes on him and make sure he ate dinner. Cause they really don't care!!

These patients are human beings not numbers you see on your chart . Treat others as you would want to be treated or a family member treated .NO matter what you made my confused mother sighn you neglected her you knew the procedures you did not inform us of any piece of paper . You knew she was confused . WE VERBALLY SAID SO MANY TIMES . And you witheld a bi pap because a confused woman said so .Because of you she is back on the hospital again , I hope you can sleep at night . because i cant .I wouldnt let these ppl watch my dog much less my mother ever again .

This facility does not even deserve the one star. As soon as my mother arrived at their rehab, the staff swooped in on her like vultures at 11pm on a Friday night, kicked us, her family, out of the room immediately to do their assessments. All we wanted to to is put her things away, say goodbye and leave. It's, july, and it was cooler outside than inside. The rooms are too small for 2 beds, even the nurse had a difficult time squeezing herself between the bed and wall, to assist with assessments. The bathroom was super tiny , and was shared by the room next door. There were NO grab bars at the toilet area, and the 2nd day mom was there, she fell. Nothing was done to accommodate the safety precautions. My mom was ordered to be on a bi pap machine nightly, and the nurses neglected to do so after my mom had vomited in the tubing. Instead of replacing the tubing, they washed it, never put it back together, and did not put it on her at night per doctors orders. This neglect, upon being discharged to go home, sent my mother back into the emergency room, worse off than she was before she went to this facility. She was not home even an hour, and we had to call 911, and she is back at the hospital. The nurses and staff lied to us on the phone just to get us off their backs, for when we would visit, the proof was in the pudding, that NOTHING WAS DONE AS THEY PROMISED. I would not even recommend this place to a homeless person!!!!

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
