I want to THANK Respiratory Therapist Nicole "Nicki" for her outstanding service to my Dad. She utilized her skills and training to correctly diagnose my Dad's inability to breathe properly. Her assessment of his dire situation resulted in quick and timely correction to alleviate his lack of appropriate inhale/exhalation abilities. We often hear horror stories about adult care facilities. This is a hero story of a Soecialst who used her training and awareness for good. She's AWESOME. Thank you Respiratory Therapist Nicole!

My mother stayed here for respite care with hospice, she came home filthy and extremely dehydrated. She was so dehydrated she did not pee for 30 hours that I know of. There staff has no communication at shift changes and received multiple phone calls for her meds which were sent with here but no one seem to be albe to find, yet they were sent home with her. Horrible experience!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
