I was a cna there at bridgeview on day shift, some of the cna's are very lazy. The care and dignity the residents need are not nowhere in this facility. I've seen a few lpn violate the rights of the residents meds stay sitting on the tray tables. The quality of life doesn't matter in this facility your love one is healthy in the beginning, but in a month time their health decline quickly. Next thing you know you get a call telling you your love one is in the hospital. The only floor the D.o.n cares about is the 3rd floor where your love ones protection from falling out the bed is no concern. The only concern is that they don't want blood on their newly remodeled floor. Where there's no mats or signs stating that these people are roll or fall risk the only floor with new beds . I was discharged for reporting a lpn taking the rights of a resident. And when you visit your family members they talk about you and them. Only a person who doesn't do their research will allow their love ones to be admitted to a abusive facility. DON'T PUT YOUR FAMILY IN HERE. SOME CNAS WONT PUT THEIR JOB ON THE LINE TO PROTECT YOUR FAMILY LIKE I DID. YOU HAVE BEEN WARNED!! The only way they do their job without end is because they know you are a regular visitor. This place rates your love ones on money not being a human being.

Compare The Number of Problems
Types of Problems at Nursing Homes
Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.
Residents Affected | |||
Severity of the Deficiency | Few | Some | Many |
Immediate jeopardy to resident health or safety | J | K | L |
Actual harm that is not immediate jeopardy | G | H | I |
No actual harm with potential for more than minimal harm that is not immediate jeopardy | D | E | F |
