    This facility is nasty, the housekeeping is horrible. The majority of the staff have bad attitudes. The food is for survival only and generally bad overall. The residents are treated poorly. They allow residents who should be watched all times roam around, the entire building, this includes other residents room while their sleeping. The pharmacy is slow in bringing needed medications. The shower room has chairs smeared with feces that is not cleaned. The nurses discuss confidential information openly with other staff. They never have clean bath towels, to shower with. The dining area is overwhelmed with the smell of urine. The elevators don't work properly, sometimes you have to wait 10 minutes! They have no structured groups for residents. The activities listed on their calendar almost never take place. They are extremelyUNPROFESSIONAL. It's like a nightmare come to life! This facility should be closed down, their always short staffed and some staff members for overnight shift come in high off drugs. They don't do body or belonging checks when you are admitted, which puts everyone in a potential dangerous situation. This place dose not deserve any star...Lord help them. The conditions are terrible to live in. I wouldn't allow my dog to stay at community care for a night. I feel sorry for the residents who don't have an advocate and they can't speak up for themselves. The state should be ashamed of themselves to keep the doors opened to this nursing facility!!!

    This is the very worst place for families in need of QUALITY care. These people WILL NOT take care of your loved ones.... They will be neglected to the fullest extent. The administrator and DON are unprofessional, dishonest and are fully aware and participate in "1-star facility behavior." The nurses and CNA's are incompetent and lack compassion; administration is well aware and turn a blind eye. Residents are getting hurt here. Please do your research. It's a dark, dank, unkept "residential outhouse." It reeks of urine that you can smell from the outside... Very sad. RN-BSN

    Hello. I have worked at Community Care for 2 years. I resigned on July 14, 2017 due to the Director of Nursing being unprofessional when it comes to her relationship with her Cna's. I truly will miss working with the residents. I care for everyone living there. In time, I will try to return back by CREATING a ministry that will Target on how the residents can cope with their unacceptable living conditions.

    This place is really bad. They're always under staff which makes it bad for the residents. The CNA's are extremely lazy and rude and the nures are no better. This place even runs out clean sheets. Residents sit around full of urine and BM. When you enter building, all you smell is urine. They NEVER serve hot meals, cold everything. The state needs to pop up on this place.
