My Dad had got transferred here from the hospital and we are working on getting him out ASAP. This place is extremely dirty, tile coming off the walls, the washrooms look like no one cleans them as there where mess all around the seat. Dirt in the corners the elevator tracks was filled with dirt and trash. The same piece of candy that was on the floor when he got there was still there the next day. This place is a joke, as you would think it would be very clean and sanitize but it was the complete opposite. Not to mention the Day room tables where full of trash. It's completely outdated, I can literally go on and on about this place but I'm going to leave at this, I would never send a love one to this establishment. Some of the workers where nice and some well act as though they didn't want to be there. If I could give zero stars I would!!!

In need of a update ASAP!!! THE CNA's are horrible there's a few that's ok though. No one cares about the patient's. They sit at the nurses stations chit chatting while patients are in need. I heard one CNA tell a patient's she just gone have to wait... ur not my problem. Really! Another patient was sitting in her wheel chair at the nurses stations and was leaning over till she was falling out and no one was paying attention to her...I had to bring it to their attention. Like they have to do better these are human beings that needs help not neglect. Treat people like you would want someone to treat you... that's all! God bless these patients they in need of better help.

This place is horrible. Thank God we were able to transfer my grandfather out of here before any permanent damage was made to him. Terrible nurses and CNAs.

Went for the first time for an evaluation. The male physical therapist I had was wonderful. He did manual pressure to my back and I'm wondering if that's the reason why ALREADY my back is having less spasms. I may be speaking too soon but maybe it's a coincidence. Anyway, I look forward to going back for therapy.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
