My husband has been here for 2 months. The care has been great. It may be an old facility but don't judge a book by its cover. If it weren't for the caring staff my husband wouldn't be standing today.

I would like to say the last place I would like to be is in a home not able to sit up walk stand nothing but guess what I did. Three months ago I was placed in collinsville rehabilitation center because I spent awhile in a comma then 3 months laying in a hospital bed. I was placed here because my insurance wouldn't pay for anywhere else so they took me in and Javita and her staff worked with fighting insurance and got it going while never missing a beat the therapy staff Chris Angela and Hailey all jumped in and pushed me to believe I could stand and walk again which after only a couple of visits I stood then a few more I took a step and as of today i am walking.i am walking after 8 months I owe them alot I also was fighting a open wound on my back witch was taken care of everyday by the wonderful nurses and thank you to the wound specialists aka dr. Headlight all of this was possible because of the bests cnas in the business. And the leader of the ship mrs. Lawanna

My brother lived here for 2 years. The administration is very unprofessional. There was peep holes in the public restroom. I read an article from BND stating a nurse didn't apply CPR to a patient so she died. They also received major infractions because their fire doors and sprinkler systems didn't work. The food was terrible. They do have some very good employees...not all. The administration is terrible.

Don't let loved ones go there. Staff isn't nice. Food is horrible. Indoor ramp isn't ADA compliant. I had a room with no bathroom or sink. Plaster falling off the walls. I transferred to willow creek and it is so much better.
