    I served as Chaplain of the former Ballard Rehab. & Nursing during the time it was owned by Presence Health and considered it to be an excellent facility. The next owners unfortunately did not uphold those standards. I am pleased to say that the current owners operating under Landmark Nursing & Rehab. have greatly improved the level of care and the appearance of the facility. I again regularly visit to offer spiritual care to Residents who request my services. Deacon Jack Smith

    My mother was recently at the Landmark of Des Plaines and we had nothing but positive experiences. The place is under new ownership /management and everyone was wonderful to us during my mother's time there. I would highly recommend this nursing home to anyone I know. The nurses provide competent care and go the extra mile to make sure the patients have what they need and they also go the extra step for the families as well. The building was very clean and my mothers room was rather large. They also have a nurse practitioner in the building that came to see my mom when she wasn't feeling good. My mother also had wonderful CNA's. The rehabilitation services helped her get stronger after her stroke! My mother rehabilitated well and luckily came back home to us. Thank you Landmark for the love, care, and compassion you showed to my mother and I.

    All of you who keep complaining about Ballard the reason why you people treat workers like animals first if you have family members in a place where they can't do nothing for themselves try not to be disrespectful to workers specially CNA who's cleaning your in-laws, mother's, father's, when you treat workers like nobody they not gonna be able to show you hospitality and stop been pushing workers cause the problem sometime is you family member who don't appreciate and never satisfied I bet if it was that easy to take care of those patients at home all of you would have done it please stop complaining about workers start appreciate instead and youlyo the difference between Love and hate

    Years ago my mother went to ballard for rehab and enjoyed her stay. This time around it was horrendous. She ended up with a bed sore, but drs and nurses refused to accept this. They said she was causing it herself by moving around in her wheelchair. My mother has been in a wheelchair for almost 2 decades and has never caused herself a sore. Within in 30 days in the facility she lost 20% of her body weight. She was not receiving her meds at appropriate times which caused multiple issues. When trying to check her out we were informed that if she was checked out without approval we would be barred from checking in to any facility in the future. We were finally able to rush an approval and remove her from this hell hole. Do not send your loved ones here.

    I'm only giving it one star because I have to. This is the perfect place to leave and forget someone you claim to love. Way understaffed. No customer service skills at all. No hospitality. The place smells like a dirty diaper. Not sure what they serve, but they claim it's food. This place should be closed down.
