CORPORATE!! STOP!! USING OTHER PEOPLE TO DO YOUR DIRTY WORK. STOP HIDING BEHIND YOUR, ADMINISTRATORS YOUR SOCIAL WORKERS AND YOUR DIRECTORS... THIS IS WHY ONE DIED.. TO MY REGRET... 3 day's after your Elgin Facilility went under investigation for abuse by your nurse coordinator.... WITH ALL THE UNSPOKEN STRESS Aperion CARE IS RESPONSIBLE FOR , AND ONE WAS SUSPENDED FOR LACK OF MENTAL HEALTH TRAINING... SINCE MY LAST REVIEW 1 OF THE ONLY FEW GOOD Men here at Apreion care 134 N. Mclean Blvd HAS PAST AWAY....MR JIM Administrator, We Miss you dearly. So! here it is... September 18th 2018, About 3 WEEKS After THE STATE WAS NOTIFIED ABOUT THIS FACILITY AND IT'S POOR PERFORMANCE & CUSTOMER SERVICE. 30 day's or so after the planned discharged date i wanted to be able to leave the facility, after Numerous phone calls, Appointments, looking for housing, not just any housing! special needs housing, housing that i can function in & properly, SAFELY & SECURELY, IN THE MIST OF LOOKING I CAME ACROSS A PROGRAM CALLED ...(AIM)...it is a program that assist individuals who have been in a nursing facility 90 consecutive day's or more, with no missing DAY'S..........THANK GOD.... FINALLY SOME KIND OF HELP. THIS KIND OF INFORMATION IS CRITICAL FOR RESIDENTS IN THE FACILITY , WHO CAN STILL FUNCTION IN THE COMMUNITY. I WONDER WHY THIS FACILITY WAS NOT AWARE OF THIS INFORMATION! I AM STILL BEING Harassed about my disability check when one had to pay for the deposit and the next one has to pay the first months rent !!! THE REASON WHY I AM TAKING THE TIME TO SAY ALL THIS IS....WHAT IS THE NURSING HOME'S TRUE AGENDA?? IS IT TO SEE ME RETURNING TO THE COMMUNITY OR STUCK IN THEIR NURSING HOME COLLECTING 30.00 DOLLARS A MONTH OUT OF MY HARD WORK ALL THE YEARS IN MY LIFE....HMMMMM... JUST WONDERING. I ASKED THE SOCIAL WORKER WHO AND WHY SHE KEPT COMING BACK AND FORTH , SHE SAID CORPORATE, WAS PRESSURING HER TO GIVE THEM MY CHECK OR GET OUT.... WELL I TOLD HER AGAIN I AM LEAVING... AND THEY CAN NOT HAVE MY CHECK... BECAUSE IT IS NEEDED FOR HOUSING... I HAVE ASKED FOR A ITEMIZED BILL OF WHAT I OWE THEM SO THAT I CAN PAY THE ALREADY PAID BY MEDICAID & AND IF THEY BILL MEDICARE PAID FOR JUNE JULY & AUGUST,AFTER GETTING FROM THE HOSPITAL PROOF THAT I WAS ADMITTED TO THE HOSPITAL AND NOT UNDER OBSERVATION, WICH WAS THEIR RESONING FOR NOT BILLING MEDICARE FOR THE OTHER PORTION OF MY STAY.... WOW... I REALLY AM NOT AGAINST PAYING FOR ANYTHING....BUT DON'T CHARGE ME DOUBLE ..JUST BECAUSE I NEEDED A EXTRA 30 DAYS' TO SECURE SAFE HOUSING TO ME I SHOULD ONLY BE LIABLE FOR ANYTHING AFTER 90 DAY'S .WHEN I SAY DOUBLE...CHARGING MEDICAID FOR 90 DAY'S AND IF I AM IN THE FACILITY 1 DAY OVER 90 DAY'S, CHARGING ME AGAIN FOR THE SAME MONTHS THAT WAS PAID BY MEDICAID & AND WOULD BE PAID BY MEDICARE IF THE BILLING WAS DONE PROPERLY..... IT SEEMS TO ME, NOT EVEN BILLING DOES THEIR JOB PROPERLY... WELL I CONTINUE TO HAVE WHAT BILLING NEEDS TO BILL MEDICARE..... IT SEEMS THAT THEY DO NOT CARE MEDICARE CONTINUES TO TELL ME THEY HAVE NOT GOTTEN A CLAIM FROM APERION CARE... OH WELL THE NURSING HOME HEALTH CARE SYSTEM IS BEING ABUSED BY THE FACILITIES, NEVER THE LESS I DO PAY MY BILLS..... DO NOT MAKE THE MISTAKE AND PUT YOUR FAMILY MEMBER IN THIS FACILITY .....OR ANY OF THEIR SISTER FACILITIES...I HAVE SPENT MORE TIME FIGHTING FOR MY RIGHTS THEN TRUE HEALING. AND TO YOU CORPORATE....IF YOU HAVE ANY QUESTIONS AS TO WHEN I AM LEAVING YOU KNOW WHERE I AM AND YOU CAN ASK ME YOUR SELF. STOP PUTTING JUST OUT OF COLLAGE NO SKILLS SOCIAL WORKERS IN UNCOMFORTABLE & HARASSING POSITIONS BY PUTTING MONEY BEFORE THE WELL BEING OF A PATIENT..... TO ANSWER YOUR QUESTION...I WILL BE OUT OF YOUR FACILITY SHORTLY WE ARE DILIGENTLY WORKING ON MY EXIT THE DOCTOR, NURSING, SOCIAL SERVICES AND THE STATE.....

WONDERFULLY!!! RESOLVED.... I MUST SAY... I AM LEAVING THE PRIOR REVIEW UP ONLY AS A EXAMPLE TO MAKE REFERENCE TO HOW A COMPANY DIRECTOR CAN BE MADE AWARE OF AN ISSUE, AND IMMEDIATELY... ADDRESSED THE PROBLEM I WAS HAVING, WITHIN THE FACILITY.HIS NAME IS MR JIM LOYD , SOME WHERE THERE WAS A COMMUNICATION BREAKDOWN.... AND HE WAS NOT MADE AWARE OF THE ISSUES I WAS HAVING,AS SOON AS HE WAS MADE AWARE OF THE SITUATION, HE IMMEDIATELY... TOOK CARE OF EVERYTHING THAT I NEEDED. AND I AM PLEASED TO SAY.. I CAN HEAL WITHOUT WORRIES... THANK YOU MR JIM. DON'T PUT YOUR FAMILY HERE....The Quality of Care Here is HORRIBLE!! ALL THEY ARE CONCERNED ABOUT IS YOUR MONEY, AND WHAT YOUR INSURANCE PAYS THE ROOMS ARE LIKE SMALL CLOSETS AND THEY CRAM 2 PEOPLE IN THEM THEY HAVE POOR VENTILATION... IN THE ROOMS THEY WERE GOING TO PUT ME IN SO I REFUSED TO GO IN THEM I NEED A HOSPITAL BED AND THEY WOULD NOT SUPPLY IT, I SLEPT SITTING UP IN MY WHEELCHAIR AND MY FEET UP. THE SOCIAL WORKERS HARRASED ME THE ENTIRE TIME I WAS THERE ABOUT MOVING IN TO ONE OF THOSE CRAMMED ROOMS THE FOOD WAS NASTY AND WHAT FOOD WAS GOOD I ONLY GOT A HANDFULL OF, I WAS ALWAYS HUNGRY. I WAS NOT THE ONLY ONE I WOULD HEAR OTHER RESIDENTS COMPLAIN ABOUT BEING HUNGRY!!! THEY DON'T DESERVE 1 STAR.

My husband has received excellent care at Asta/Arba Care. They are always concerned about his needs and has always addressed them in a timely manner. The staff are all caring. His room is always cleaned. I have always received friendly caring correspondence whenever I would call or I would get a call from them. I am always there at Asta/Arba visiting my husband, and have seen for myself the professional care my husband receives. He likes the food, too.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
