Very clean environment, although the nursing staff is helpful and caring the CNA'S are only there to collect a paycheck. They also don't keep very good track of the elderly's possesions

Heritage village has helped many people. The aids do assist people right in time. I know this because my mom works here. So, I am there all of the time. My mom and her nurses take care of every patient when they need it.

Very disorganized! Understaffed! Multiple fines from the State of Illinois this year. Over a 20:1 resident to Staff ratio. Nurses refuse to assist aids. Calls for assistance from residents go nearly an hour without response from staff. DO NOT place your loved ones at this facility.

My father was recently a short term resident in the Heritage Village Skilled Nursing Unit. He was certainly pleased in every way with his stay. My sister and I felt he was well taken care of by the RN's, CNA's and PT Department. In a less than desirable situation, the Village does their utmost best to provide compassionate, quality care. Thank you, Maass Family

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
