I came to visit Avantara Long Grove, not knowing what to expect. I was so pleasantly surprised and happy that I did! The staff that I encountered were so warm and friendly, it felt like a welcoming place. I also couldn't help but notice how clean it was - truly immaculate and no odor - which is so important to say. I saw there was some sort of event going on, and the patients there were having a good time with the staff; it's rare to see that anywhere else. I felt comfortable with the place, and compelled to share my very positive experience!

I could have not asked for better care. I arrived at Avantara a week after surgery full of uncertainty. They prepared me to go home physically and emotionally. Thank you Dot for your dedication to get me to reach out and get help at your facility! And then Jennifer for making it happen! Everyone treated with kindness and care, and always addressed me professionally. The nurses, I remember a few- Sharisha, Shaila, Merisol and there were many more, were always responsive to my needs. I will never forget my physical therapists- Kent, Colin and Carl. They always asked what I could do but never pushed me past my capabilities. Dr. Cozzi was caring and amazing. Patty, the social worker, was always smiling. I am now happier than I have been in years. Avantara made a significant contribution towards the return of my happiness to my life. Thank you all!! Hello Phyla! (H is silent)

There are numerous rehabilitation and long term care in northwest suburb of Chicago . What make Avantara of Long Grove stand apart from others it is the people who works there. Their hard work and great care for residents make this center is better place . Any concern , any question you have about your love ones they will address it right away . Since my mom became resident of long term care she has been surrounded with professional nurses , who is always making sure she is taking her meds and staying healthy, CNA, who is always there to take care of all her needs , PT and OT, working with her trying to keep her physically active. Avantara at long Grove has an amazing Russian program runs by Tatsiana who is always comes up with new ideas and fun things to do for residents. Activities aids are amazing. They are always making sure my mom and other residents are participating in their activities and having fun . Concerts , karaoke , playing with balls, manicure , coloring ...just to name a few always provided by them . Such care would not be possible at home , and as much as my heart breaks from leaving my mom there I know that she will be taking care of and I can worry less about her wellbeing.

Avantara Long Grove is one of THE BEST nursing home facilities that I have visited. They took excellent care of my mother-in-law. The room was clean and modern...the staff...just incredibly compassionate and caring. A special shout-out to the administrator Jennifer who spent time out of her busy schedule to talk to me, reassure me, understand me! Thank you...thank you! Mary Varughese

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
