My mom has been at the facility 6 times for rehabilitation . The staff are kind caring and excellent at their jobs. The staff went above and beyond to ensure my mom was comfortable and all her care needs were met. I had to go out of town during several of her admissions and I was confident that she would receive excellent care and she did! My mom never had any complaints and is always happy to return there for rehabilitation.

This place is horrible! Do not send your family members here! My mom was in there for 3 weeks and had I not been there the whole three weeks, this woman would not have gotten any kind of proper treatment. They went as far as waking her up, at 0500am the day after she checked in, to weigh her 5 days after double knee replacements!! Mind you they weighed her upon arrival not even 12 hours before. They Staff is slow, the food was horrible. Luckily about the only nice thing was, since a family member worked there they let me have a bed and stay with her. About the only good thing was her being able to walk and sit outside on the patio area.

Why should i get myself upset ......this place is not fit for a human being . once a week your allowed a bath.the floor is washed once every two weeks.. The night shift are good but the day shift is no good ..and you better mark it down when you get your pain pills or you will be screaming in pain....I did not ask to go there I wanted to go to my Hosp on Addison and central where people that work there are human beings .this is suppose to be a Catholic Hosp huh. More like a torture inhumane abuse center.no compassion or consideration .that is what rehab center I was in. And that administrator covers up what goes on you complaine and she will stick up.for her staff no matter what they do after she comes to your room in her army clothes on her horse if there are any good reviews on this place they were put there by one of the staff.after I put in a bad review two girls came to my room to try to change my mind about the bad review I had of the place I told them I Dont lie.. Its obvious they do at that place I really hate that place after what they did to me .

The admissions department works really well with community hospitals to provide support for patients requiring skilled care. The therapy department provides compassionate, patient focused therapy programs with excellent outcomes. Patient focused care programs are a priority at Presence Resurrection Nursing and Rehabilitation Center.

Don't understand where these bad reviews are coming from but my dad has had an amazing experience at Sister Bonaventura. From the staff to the therapist everyone has been amazing and attentive to my father. My father does not even want to leave because of the care he is receiving. He is leaving on Saturday since he is doing so well. Thank you to the whole staff for providing such great care to my father.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
