The staff is great here and all the attention I get here is appreciative. Everyone is here to help the clients out in the upmost and professional manner. I couldn't find a better place than Elevate Care. All the staff that I've come in contact with are all active listeners and care about my feelings and opinions of how I think the staff are.

I am currently enjoying my stay here in the facility. The staff here work hard and take good care of me. Staff and managers take the time to listen and attempt to resolve any concerns I may have.

I have been a resident at Elevate Care for sometime now. I have only great things to say about this nursing home. The nursing staff is very attentive as well as the CNAs and aids. Medication is timely and correct. The rooms are clean and there aren't smells of traditional other places. It is a welcoming environment. Food is fantastic. The therapist Any maintenance issues are handled promptly and correctly. In addition, every day there are different activities for the resident's from exercise to memory games etc. Any issues with all of the above are addressed quickly and effectively by management and the administrator Marcus Shaw which all have an open door policy. And finish up I am happy here and everyone is friendly. I would recommend Elevate Care for those in need. I personally have Parkinson's Disease, Lewy Body Dementia and diabetes (2) and use my computer with Dragon voice recognition software. Larry Harris

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
