This is my fourth time coming for rehab and I keep coming back for a reason. I rank it a 5 star especially due to the therapy. Jamie and Camelle are excellent therapist, the list goes on and on. They work great as a team. It has been nice this time around as well as the times before. I cannot forget to mention Andrea and Shirley in nursing. Also, Angel, she was my nurse the very first time I went to rehab and is still taking good care of everyone. I would wholeheartedly recommend this place because it is a good place. I'm leaving but I'm leaving with a heavy heart because I love so many people.

The physical therapy is the best I have seen in the area, which includes northeastern Illinois. They have a lot of equipment and a lot of people to help. Everybody has been really really kind and could not do enough for me. Just all and all it's been a good experience. If a know anyone that is in need of therapy I will send them to your place because this is the place to go and this is coming from me, a patient that has experienced it for myself.

This place is a train wreck! The staff does not care about the patients, they only do something when they see you coming or complain. It's dirty and smells. The pictures on the website are misleading everything in the rooms are old, used and needs to be replaced. I work in the healthcare industry and in my opinion this place needs an overhaul from top to bottom, including the staff! To rate it one star is way too much! I will call the state on this facility every chance I get until something changes! I will say it again to rate this facility one star is an over statement!

If I didn't have to give this facility a star, I wouldn't because even one star is too much! My mother was accidentally taken to this facility for rehab, and when I called to verify she was being transported to the correct facility, I received nothing but a nasty attitude! To say this staff member was unprofessional is an understatement!! When she finally answered my question, she attempted to half-heartedly apologize for the confusion but by then, I was furious! I had to remind her this was my mother, but I could tell she'll never understand until HER mother is in this situation. I feel for the patients of this facility and their relatives. If I was treated with such disdain and disrespect (and my mother isn't a patient), I can imagine how much worse it is for those who are! I hope this post will help the next reader who considers this place. My suggestion: don't waste your time...it is too valuable to deal with nonsense and mediocrity!!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
