Ya this place is terrible, my dad liked Dwayne and Mary. But when he was done with rehab and Medicare stopped paying they conveniently left him in a $300 a day room because they were waiting on a double room that he would be compatible with, I said it wasn't his fault he was in that room, that said they would call me about lowering price if room and they never called ....moved him to northern lakes.....NO THANKS AOERION!

If I could give negative stars I would.. Where do I even start. My grandfather is currently here. He's 87 years old and they aren't doing as they should. They have lost his prescriptions, let him sit in wet clothes, not changed his pants/diaper after going number 2, refused to put bed rails on his bed after countless times of him falling out of bed, got tired of him pushing the help button so they sat him in his wheel chair next to the nurse desk and let him sleep hunched over for hours so they wouldn't have to keep going and checking on him. When we've complained their excuse is that they are short staffed. I'm sorry, short staffed or not its YOUR job to care for these people. Anytime we have expressed our concern it's pretty much a laugh in our face and nothing changes. I WILL not put up with this and WILL not allow them to treat my grandfather or anyone else's family this way.

My mother is in this facility and I have caught 2 nurse aids steal prescriptions and selling them to another coworker. I am very disappointed in this behavior. I tried to speak with Sara Hankey and she laughed even after I had pictures and videos to prove all evidence and after watching them she laughed at me. To be exact two females were selling stolen scripts to a male and female coworker. If something is not done with all employees that are involved we will be taking all this information over to Angola Police Department and take care of it this way. Thank you for your concern.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
Lakeland Skilled Nursing And Rehabilitation has not received any fines, complaints, or inspection problems in the last 3 years.
