I have almost worked here for 3 years now and our staff has continued to grow into the awesome, caring, and compassionate team that it is today! The management team has changed for the better, we are much better staffed then when I first started, and our resident and employee satisfaction numbers continue to grow! We strive to make our Resident's stay, whether it be short or long term, the best experience for them by meeting their needs, wants, and expectations through our compassionate, loving, and caring staff. I'm proud to say that I am part of the Wellbrooke of Crawfordsville team and I love my job!!!

I would like to give a HUGE shout out to my place of employment Wellbrook of Crawfordsville. I have never worked for more caring and loving people in my career. They love their staff and residents like family. They strive to make our residents and families know that their well being is our top priority. I hope anyone in any kind of need would look to us first because I have been in this career it will be 34 years this year and have only been with this team a short time. But I can promise you that the time I have spent here is the most rewarding time I have spent. Go team!!! We got this!!! I love you all.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
