As owner of a DJ company, I have had the opportunity to entertain here several times and have witnessed a caring staff interact with the residents. Sherry, their activity director and Jane King are always planning fun activities. Would be my first choice for a loved one!

My mother was admitted to this facility and in the first three days of her stay they waited too long to take her to the bathroom and dhe didn't make it. She turn on the call light to get help and it was on for thirty minutes before anyone came. Last but not least she had turned on her call light for help to get into bed and get comfortable for the night and next thing she knew her night nurse found her sitting up in the chair covered up with one of her hospital gowns and it was three a.m. The call light was still on and her feet were so swollen she couldn't get her shoes on. And she is only there for s short term rehab stay!!!!

I love Heritage Park . My parents have been there for a couple of months. So far the administration and staff have been wonderful. They have everything they need in this community. I feel very at peace having them at Heritage Park

My patient had his defibrillator fire and the nurse at this facility told him "that's what it's supposed to do!". Pt was not sent to the hospital.

Compare The Number of Problems
Types of Problems at Nursing Homes
Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.
Residents Affected | |||
Severity of the Deficiency | Few | Some | Many |
Immediate jeopardy to resident health or safety | J | K | L |
Actual harm that is not immediate jeopardy | G | H | I |
No actual harm with potential for more than minimal harm that is not immediate jeopardy | D | E | F |
No actual harm with potential for minimal harm | A | B | C |
