My uncle has been in their care for a few months now and we are so extremely happy with Signature. He gets amazing physical therapy and is able to walk which is a HUGE triumph that no one ever thought would happen. A lot of the staff there seems to be new, but there are so many people that truly care and work really hard to meet his every need. We are there a lot and have never seen anything to make us doubt his care. Many of his neighbors have lived there for years- one for 16!

I heard about this horrible place on Facebook, the Administrator should be fired and the owners jailed for the abuse of these defenseless ederly people at the mercy of Lazy CNA's, pitiful that Humans are allowed to mistreat weaker Humans for sport!!! If you low lifes can't smell that a man's foot is rotting off you need a different job...I'm sick of lazy people starting with the Administrator. I hope all of you lose your jobs, I hope the place is shut down and the elderly brought to safe, caring, professional facilities with real caring humans...

*Copied from FB just figured everyone should know before they place a loved one here* Went to see my dad tonight at the nursing home, he has been at SIGNATURE CARE in Fort Wayne for along time now, and Im sharing this in hopes that you will also share this post. Because of Insurance there are limited facilities here that are equipped for his heath care needs, on several occasions I have found him with wet and soiled pants, no shoes or slippers, dirty clothes, un bathed, dehydrated, other patients sleeping in his bed, and CNAs that dont care about him or anyone else in his wing as well. I have complained, Ive made phone calls..and got a bunch of empty promises of change..today I went to see him, and before I got through the doors a Nurse (who only works weekends) stopped me and told me about a sore she found on his foot, she said she cleaned it up..wrapped it up and apologized to me because she didnt understand why it had not been seen or addressed by anyone else, after seeing this, I would also like to know how it got past anyone...let alone by seeing it...but the smell made my stomach turn!!! Someone had to have noticed it...he had not been eating, his color was pale and the man is in pain, he was sitting in a wheel chair, (with soiled pants again) very out of it...he didnt talk or communicate at all, clearly he was in pain and the CNAs working acted as if they were to busy to do anything to make him more comfortable, my point of putting this on facebook is to get the word out about this place..dont send a parent or loved one there, they will not get the care they need, my dad barely remembers his name let alone remembers to go to the bathroom and if your only taking a CNA job for a paycheck, please dont take this job on..you have to have a heart for this job, you have to want to be something or someone better than the world, your needed on so many levels and its bigger than a paycheck. So please pay attention to these elderly people who need you, their lives could depend on it!!! STAY AWAY FROM SIGNATURE CARE FORT WAYNE!!!!! Located off State St. And Maplecrest Rd.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
