    If you love your family, don't send them here. My father almost died because the RN said he likely had a UTI instead of the Pulmonary Embolism which he was diagnosed with in the ER, after my mom ignored the RN. The physical therapy folks were good, but if the nursing staff is incompetent, that doesn't really matter.

    The facility is new and clean but do not let this fool you. My Dad was admitted there for rehab and the only positive thing I can say is the therapists were good. The lack of care outside of rehab was unacceptable. They forgot to give him his medicine twice. He would not have received if we hadn't insisted that he be given it. He was left in his room after a meal on portable oxygen and they did not hook him back up to the condenser. The portable oxygen ran out and I found him hours later without it. Another time they brought him back to his room and did not connect his oxygen again. Five hours later I found he wasn't connected and called the nurse. She told me that she wasn't the one that did it and that I didn't need to call her about it because I could do it myself. She even refused to check his oxygen level after being without it for 5 hours. Also my Dad asked for a nurse at 4am and he was told that the nurse "stepped out" for 4 hours. It was 8am before a nurse came and that was only because I called an demanded that a nurse see him right away. An aide refused to empty his urinals because she said she was sick of doing it and he would have to do it himself. Mind you he was not able to get out of bed without assistance. He was never given a shower in the total of 22 days he was there, we had to bathe him ourselves. His sheets were never changed, we had to do that ourselves. No one should be in this facility unless you have family or friends monitoring your loved one 24 hours a day. The lack of care and negligence was appalling. We did address these issues with management right away and nothing was done.

    I've worked in several nursing facilities prior to coming to this one. Never before have I met such amazing staff. The people who choose to come to work here pour their soul and heart into taking care of their residents. Everyone from Dining Services, Environmental, Nursing and the rest of the crew come to greet you with a smile and won't hesitate to stay with your mom or dad to just hold their hand. I feel blessed to be a part of these wonderful caregivers.

    Family member 95-year old resident on assisted living side. She was transferred to rehab side for post-op nursing care and therapy. (Post-op nursing care was poor at best. One aide refused to change surgical wound dressing stating "it's not my job.") Resident returned from dining room Friday p.m. 2-1-2019, pushed into room, aide stated "now you're on your own." Resident on portable O2 for travel to dining room. Nurse/Aide failed to check O2 level on portable unit upon return to the room and failed to switch resident to room O2 concentrator. Portable ran on empty until nearly midnight when paramedics were summoned for a non-responsive resident. Her O2 level at 57%! ER medical staff were seriously concerned as to how long resident was with out proper O2 therapy. Cost continues to increase as the responsive care declines. We have fami ly who work in the health care industry. They are concerned for the residents at WellBrooke of Avon.
