It was a pleasure to help and Minister the residents. It brings me joy to see such helpful and motivated Staff members to rush to the care of their patients. It's an honour to know this Facility as hard working C.N.A. and Nurses who daily provides a safe, clean, hot food, enjoyable activities to bring peace and restore harmony with a humble and hospitality attitude services. Keep up the Great work!!!!

Worked at this company when first open and did not stay long, worked 3rd shift and would take pictures of employees and nurses sleeping on job. Two years later went back to work (which was 3wks ago I left) and nothing changed worked 3rd shift the CNA would come in at 10-11pm and would not check anyone or do any rounds or pass ice water, tginknof your mother not having water or being changed until 4 am. They never had any towels available to clean anyone so the residents would not get washed. I listened to a nurse (Erin) talk so bad about a resident that she didn't know I personally knew about how they need to learn to get up and walk but he couldn't. Also this same nurse was told someone's father fell on the floor and she never reported it. I would definitely check this place out more then once and do several pop ups before I ever think about putting my loved one here.

I WAS PUT IN THIS REHAB FOR THERAPY DUE TO AN INJURY TO MY BACK NEEDED INTENSIVE THERAPY. THE COUPLE DAYS I DID WORK WITH PT/OT THEY WERE GREAT IT'S THE STAFF THAT ARE HORRIBLE AT THIS FACILITY. I WAS LEFT ON THE BEDSIDE COMMODE FOR SO LONG SO MANY TIME'S I WAS FORCED TO CALL OUT SIDE OF THE FACILITY SO SOMEONE CAN CALL BACK IN AND LET SOMEONE KNOW I WAS ON THE COMMODE MIND U I'D ALWAYS PUSH MY CALL LIGHT.I WAS TREATED VERY AGGRESSIVELY BY ONE OF THE NURSES.AFTER A SEIZURE I WOKE UP IN URINE.THERE SO SHORT STAFFED THAT THE ONES THAT DO WORK GET SLAMMED WITH EVERYTHING. MY WIFE HAD TO RUSH ME OUTTA THERE TO THE HOSPITAL BY AMBULANCE BECAUSE THEY CONTINUED TO DENY ME TREATMENT N I'D GAINED OVER 30 POUND'S OF FLUID IN LESS THAN A WEEK. I KNOW MY BODY BEEN LIVING WITH LUPUS AND ALL THE REST FOR QUITE AWHILE NOW .I'M CURRENTLY HOME N HAVING ALL THE FLUID REMOVED. AND NOW I CAN BREATH.THERE ARE SOME VERY UNPROFESSIONAL DR'S AND STAFF THERE.AND THEY'LL LIE TO TRY TO SAVE THERE SKIN.IM GLAD I GOT OUT OF THERE.I JUST WORRY ABOUT THOSE BEING MISTREATED. LIKE A LADY WITH DEMENTIA THAT SCREAMS FOR HELP ALL DAY AND NIGHT. MAY GOD BE WITH THEM ALL.

This place called my father a cripple to his Face. One of the girls challenged him to a fight. My Father is 86 years old. And When I took him out of this place, They stole his electric razor that I just got him.. And they stole other brand new electronics valued at 150 and 100 dollers! And I reported the abuse to Ted Kenedy "Claims hes the main person" And he did not do anything about it. That is why we pulled him out of this hellish place!

This Faulty, of is unfit to be in the staff, is unprofessional, do not that care of the patients, do not answer the lights, of the patients, the State has investigate, but didn't say anything, to the families of the patients, make up lies, to the families, about treatment and abuse the families, giving the time to go and come, etc 1/2 hour for visitoration,in the morning and the evening, The Owner should be aware of this behavior, the nurses taking into their own hand to do this to the families, this is awful treatment, for a New Nursing Home Faulty

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
