I am currently at Plaza West recieving rehabilitation care. I have visited a few other care facilities and Plaza West rates as one of the best. The nursing staff is easy to work with, caring, encouraging. The therapy staff keep things fun and interesting. Then there's the food. The menu needs updated to include healthier adult options, seasonings would be helpful. Overall, Plaza West makes being away from home somewhat pleasant, like an extended family.

This is an absolutely wonderful place for rehab . This is my mom's second time being there and she is treated with respect and compassion . The employees are super sweet and very attentive. I will recommend this place every time.

Updated December 9th, 2019 I'd hoped that I would be coming back to write a review in which I can say the issues with Plaza West administrative staff have improved, but I cannot. I have increased this rating by one star but that is only because of the care staff commitment to residents. As of this date, the billing issues are my biggest headache. If you plan on using Plaza West be prepared for issues with billing. There have been billing issues every month since my first review. The care staff who have direct contact with residents are very nice, they are very attentive to the needs of the residents and respond well to the families and friends of the resident. My friend is happy with his level of care. He enjoys the outing and activities. If I was just reviewing the personal care staff, I'd have given 4 stars. June 19th, 2019 I can not fully comment on the care of residents, but I can comment on the responsiveness of the administrative staff and the issues with the intake process. I went in a spoke with about the possibility of accepting a new resident, a long-time friend of mine. I spoke with David Taylor and it was my understanding that we had started the process of accepting a new resident. David asked several questions about insurance, payment, what we might need and we discussed the cost. He showed me and another person around, explained the activities, dining area and recreation that would be available to my friend. He indicated that an intake assessment would need to be done and that he'd get that started and get back to us. We explained that my friend was ready to move at any time and time was of the essence. With the process of accepting my friend, I felt that David was handling it. At least that is what I was led to believe, but David then went off on vacation without completing the intake or speaking with us. The week after I went in to speak with David and starting the intake process I had not heard anything back from Plaza West. I called and was informed he was on vacation. But that Matt Harman the Long Term Care Administrator would be happy to help me. This began the process over again and it did not go any better than with David. By both David and Matt dropping the ball I did not receive the impression that my friend was important. The social work staff were very nice and explained about activities and did a questionnaire about what my friend liked, what his hobbies and life had been like be for needing care. Once the social work staff was involved, things did start to move along. This is my first experience with Plaza West and again I can not at this time comment on the care of residents. If their administrative staff are any indication I am sure that any information added to this review about care will is unlikely to be positive. The front desk staff is friendly and receptive to helping. They are understanding and do their best to get the right person to speak with and help resolve questions. At this time I am not able to comment on the business office as I have not had time to make contact with them directly and to complete the paperwork required by the business office. I'm hoping that they are as friendly and receptive as the front desk staff and social workers. Plaza West does have a hair salon on-premises, the dining room is large and centrally located. The front door of the main building is monitored to ensure the safety of residents. I have not had the opportunity to meet with the nursing staff for any length of time, therefore I am unable to comment on their level of care for residents. I would advise anyone considering Plaza West to know that it will require you being extremely proactive and require you to follow up on every step of the process to ensure that the administrative staff does not drop the ball. I will update this review in 2 months and hopefully, I will be able to state that my opinion of the administration has improved.

This place is ridiculous! Let me give you some certain "allegations" why this place is poor and upper management doesn't know what the hell they are doing. For one.......you guys don't have people skills to hire the right people for the most part! You have people in there lying about almost anything they can lie about and making things up. For two......the place is dirty!! Not a real good "healthy" environment for most! For three.......Poor UPPER Management!!!! Whats one thing you do to make that place better?? I'll wait............Just know, this will not go being not known. Believe that.......And BTW the kitchen was disgusting.......smh.....

Excellent Atmosphere, excellent staff, super cleanliness, and happy residents. They responded quickly when my partner started showing signs of a stroke. Their quickness, kept him from having long term effects. I highly recommend Plaza West for loved ones. (I was scared of the reviews) What a wonderful surprise when I visited. Thank you for taking care of our loved ones!!!

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
Plaza West Regional Health Center has not received any fines, complaints, or inspection problems in the last 3 years.
