S.B admitted to Rolling Hills for skilled rehab in August 2019 after having an experience at anther Community that was not so pleasant. Family agreed things weren't always perfect, but compared to where they had been the communication was much better and they were seeing the great progress mom was making. Thank you Rolling Hills for making the difference.

I have been at Rolling Hills for approximately 3 months serving as Director of Nursing. Mission Health is a company that treats the staff and residents as part of their own family. They stand behind the Mission and Care values. If you are needing great care for a family member or loved one reach out to us and let us be your resource. If you are looking for a positive career change we are the place to look.

Not only have I made the career change to be the Assistant Director of Nursing here at Rolling Hills a Mission Health Care community, I moved my father from another community, because I believe in the care and love shown to our residents is top notch. Mission has great standards which drives awesome employees providing great care.

Love working with residents and their families, no two days are the same and I wouldn't want it any other way. Our residents are our livelihood and we work in their home. Each day is and should be focused on the person as an individual. Come home to Rolling Hills!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
